I/We, the parent(s) or legal guardian(s) give my/our consent for my/our child (the participant) to attend and participate in Wonder Juction Vacation Bible School activities offered by Enola First Church of God, located at 9 Sherwood Drive, Enola, pa 17025, from June 22nd thru June 26th, 2025.
In addition, I/we give my/our consent to have my/our child's (the participant's) picture taken during Vacation Bible School for the purpose of a video presentation to be shown during the Vacation Bible School closing program and worship service held in the Church.
Lastly I/we give my/our consent for the Vacation Bible School nurse/medical staff to administer basic first aid to my/our child (the participant) in the event of an injury and I/we understand that no medications of any kind will be given to my/our child(the participant) by the nurse/medical staff unless the child (the participant) has a prescription for which he/she has the actual bottle that contains patient's name , dosage information & directions with him/her. I/we understand that the Vacation Bible School nurse/ medical staff will contact emergency services (911) , in addition to the emergency contact listed on the registration form, in the event of a significant injury and agree to pay for all expenses for such emergency services.
